CPT 29806 represents Arthroscopy, shoulder, surgical; capsulorrhaphy. This is the code used for repairing a SLAP (Superior Labrum Anterior to Posterior) tear.
The SLAP vs Tenodesis Conflict
In many older patients, surgeons will perform a biceps tenodesis (29828) instead of, or in addition to, a SLAP repair (29806). Medicare NCCI edits strictly bundle 29806 and 29828. If both are performed, you generally only bill the more extensive or clinically dominant procedure (usually 29828) unless performed on separate areas of the shoulder with distinct incisions.
Medical Necessity
Ensure the operative report clearly defines the type of SLAP tear (Types I-IV). Type II and Type IV tears are the most common indications for surgical repair.
NCCI Edits
If the SLAP repair is performed at the same time as a rotator cuff repair (29827), they can be billed together, but Modifier 51 (Multiple Procedures) or Modifier 59 may apply depending on the payer.
