CPT 93458 is the gold standard code for a Left Heart Catheterization that includes coronary angiography and left ventriculography. Because it is highly comprehensive, it has strict bundling rules.
What is Included in 93458?
This single code includes:
- Introduction of the catheter into the left heart.
- Injection procedures for coronary angiography.
- Injection procedures for left ventriculography.
- Imaging supervision, interpretation, and report.
Bypass Grafts (93459)
If the patient has previous coronary artery bypass grafts (CABG) and the physician selectively catheterizes and injects those bypass grafts along with the native coronaries, do NOT bill 93458. You must up-code to 93459.
NCCI Edits and Interventions
If a diagnostic cath (93458) reveals a severe blockage and the physician proceeds to place a stent (92928) during the same session, you can bill both, but you MUST append Modifier 59 (or XS) to the diagnostic cath code.
