When an office visit takes exceptionally long, standard E/M codes aren't enough. CPT 99417 is an add-on code used to capture prolonged outpatient E/M services.
When to Bill 99417
CPT 99417 can ONLY be billed in conjunction with the highest level E/M codes (99205 or 99215). It is billed in 15-minute increments once the maximum time for the base code has been exceeded.
- 99215: 40-54 minutes.
- 99417 (First unit): Can only be billed when total time hits 55 minutes.
Medicare Exception: G2212
It is critical to note that Medicare does NOT accept CPT 99417. For Medicare patients, you must bill the HCPCS code G2212 instead, and Medicare's clock starts much later (you cannot bill G2212 until 69 minutes for an established patient).
Add-On Code NCCI Edits
As an add-on code, 99417/G2212 never requires a Modifier 51 or 59 when billed alongside its primary code (99205/99215). Check your edits if adding additional procedures.
